Software Installation Request Form

TWO WEEKS LEAD TIME REQUIRED

This Installation Request Form is for any special software installations needed by instructors in the University ITS M037 PC Training Room. This software must be for instructional and training purposes. Two weeks lead time is necessary for preparation of software installs. Instructor must supply the software and installation instructions, and have the proper license for all copies. (Training room contains 24 machines for students and 1 for instructor.)

This form can be filled out for the Software Installation Request and submitted online through the World Wide Web. You must use a forms-capable web browser to correctly complete this form.
